Coach and Manager Self Nomination

Coaches and Managers are required by GHFA and Eastwood St Andrews AFC to provide their full details to receive their Photo ID.

In addition to their details, as a Coach or Manager, you will be required to complete a Prohibited Employment Declaration (form available here).

Before filling in the nomination form below you need to have the following ready on your system;

1. Prohibited Employment Declaration form completed and ready in either PDF or JPG format.
2. Proof of age/ ID i.e. Drivers licence, passport, etc... in PDF or JPG format
3. Photo of nominee - 'passport style' (head and shoulders with a blank background) in JPG format
